What Counts as “Chronic” Low Back Pain?
Doctors generally define chronic low back pain (CLBP) as pain lasting 12 weeks or longer in the lower back, often accompanied by pain that radiates into one or both legs. It’s an extremely common problem: global prevalence estimates put it somewhere between 13% and 20% of adults, and the number of people affected has grown substantially over the past few decades. Chronic back pain doesn’t just hurt. It can lead to muscle spasm, weakness, and a gradual loss of the trunk stability that normally protects your spine, which is part of why it tends to become a self-reinforcing cycle that’s hard to break with rest alone.
What the 2023 Meta-Analysis Found
Researchers pooled 12 randomized controlled trials, the gold standard for clinical research, comparing ESWT against a range of standard conservative treatments: exercise therapy, physiotherapy, trigger point or steroid injections, oral medication, and manual therapy. In total, 632 patients were included, 318 receiving shockwave therapy and 314 in various control groups.
Pain relief. Patients treated with ESWT reported significantly less pain than control groups at both 4 weeks and 12 weeks after treatment began. The improvement wasn’t just an initial bump; it held up three months out, which suggests the effect isn’t purely short-term.
Function and disability. Using the Oswestry Disability Index, a standard questionnaire measuring how much back pain interferes with daily activities like walking, sitting, and lifting, patients in the ESWT group also showed significant improvement at both the 4-week and 12-week marks compared to control groups.
Safety. None of the 12 trials reported any serious adverse reactions related to shockwave therapy. A small number of patients experienced mild, temporary soreness or redness at the treatment site, consistent with what we typically see in practice.
Mental health. Worth being upfront about: the research did not find a significant difference in mental health or mood scores between the ESWT group and control groups. How Shockwave Therapy Works for Back Pain
Extracorporeal shockwave therapy uses focused acoustic (sound) waves delivered through the skin to the affected tissue. Researchers believe it works through a few mechanisms acting together:
- Pain signal interruption. Energy from the shockwaves stimulates pain receptors in a way that appears to block or dampen how pain signals travel to the brain.
- Improved circulation. The mechanical forces from the waves promote blood flow to the treated area, supporting the body’s own healing processes.
- Reduced inflammation. Some research suggests ESWT lowers levels of certain inflammatory markers in irritated tissue.
The exact biology is still being studied, but the pattern across this research and the broader ESWT literature is consistent: less pain, better function, and a favorable safety profile.
Focused vs. Radial Shockwave: Does the Type Matter?
Not all shockwave devices are the same. Broadly, there are two categories: focused shockwave therapy (f-ESWT), which concentrates energy precisely at a specific depth, and radial shockwave therapy (r-ESWT), which disperses energy more broadly near the surface. The 2023 review notes that other studies have found focused shockwave to outperform radial shockwave for certain conditions, such as rotator cuff tendinopathy and knee osteoarthritis, though evidence comparing the two specifically for chronic low back pain is still limited.
